Remove namespace std from fixed length integer types

Florian Uhlig requested to merge f.uhlig/cbmroot:remove_namespace_integer_types into master

This becomes possible after using the cstdint header everywhere.

This supersedes !2145 (closed) and reorder and squash the commits from !2145 (closed) such that there are only two. One with the changes and one with the formatting.
