Cleanup mvd code

Update license header.
Use nullptr instead of macro NULL.
Remove path from include statements.
Add missing include statements and remove unneeded ones.
Use forward declaration whenever possible.
Remove obsolete functions.
Move cout to LOG.
Remove some unnecessary code.
